To purchase health benefits:

1. Decide which insurance company and plan is best for you.
2. Fill out an application form. You can choose an online application (most of them allow you to enter data online), or download a form that can be filled out and sent in at a later time. See "Application Forms" below (we can also mail you an application if you desire).
3. Mail or fax the completed health insurance application to either us or the insurance carrier (depends on the carrier), and:
4. You may also have to include a cheque (payable to the insurance company) for the first premium installment, or a void cheque, etc. This will be outlined in the application form.
5. If approved you will be mailed your insurance policy*. Usually coverage commences on the first day of the month following approval of your application.

* You will be notified if not approved or if your monthly premium has been adjusted and is not what your were quoted. If your monthly premium has been adjusted you have the option of refusing your policy (and if you have mailed a cheque you will receive a full refund).
